EMA EMR to OFA 3 Application
Candidates in possession of a current Emergency Medical Assistants (EMA) License may apply for an Occupational First Aid certificate at the appropriate level from an authorized training agency.
A licensed Emergency Medical Responder or higher level of EMA licensure (EMR through CCP) may be issued an OFA 3 certificate. As EMA Paramedic licenses must be maintained annually to remain valid, an OFA certificate issued as a result of this EMA EMR to OFA 3 Application is valid for three (3) years from the date of application.
You will be required to complete the application package, as well as present your photo ID and EMA FR license.
Application Process
To complete your application, you will be required to:
- Complete the OFA Out of Jurisdiction Jurispruduce Package
- Fill out and sign an OFA Statement of Fitness
- Fill out and sign an OFA Out of Jurisdiction Certification Application
- Present your photo I.D.;
- Present your current EMA EMR license.
